• US Lacrosse Form [pdf] (NEED 2 COPIES)

  • Please do not send the US Lacrosse Form directly to US Lacrosse.

  • Send the 2 copies with the Registration form to the address below.

  • Do not send a separate check for the US Lacrosse registration, it is included with the WYLL registration fee

2.    Make check of $150 payable to:

               Walpole Girls Youth Lacrosse
                add $185 for each additional boy sibling player
                add $135 for each additional girl sibling player

WYLL wants to encourage all youth to learn and play lacrosse. If there is a financial challenge that may prevent a child from playing lacrosse the WYLL Board of Directors will (as appropriate) waive the User Fee and also (as appropriate) supply equipment.
